• Child’s Birth Certificate
• Immunisation History Statement from Medicare

The following documents are to be submitted if applicable:

• A Parish Priest Reference form to be completed by your Parish Priest (if applicable)
• Kindergarten Student Health History Form (Kindergarten students only)
• Baptism Record (if applicable any denomination)
• School reports
• If student and/or both parents were born overseas, a copy of student and parents Visa, Passports or
  Citizenship please

A $55 non-refundable application fee is due per student on acceptance of a position.
